RxJava and Postgres Based Reactive Work Queue

I'm building a work queue backed by a PostgreSQL Table.
I have an Observable of INSERT events into this table. Subscribed to it is a function that will process a single item from from the table, and removes it from the table.
This works with all new items inserted after subscription.
Since the table is persistent, the table may contain items for which we will never get notifications. (For example left-overs from non-graceful shutdowns.)
How could I solve this problem best with RxJava?
I saw there is an Observable.startWith method:
observableOfInserts.startWith(observableWithCurrentDbItems).subscribe(processSingleItem)
I'm not sure this gets the coordination of what I want right?
